Whatever it takes to help you maintain your independence – from maintenance jobs like mowing the lawn or cleaning gutters to personal services like transportation and household bill-paying – HomeWorks will help you find the right people for the job, so you can continue to live at home as long as possible.
The LifeStyle Solutions Plan's local service affiliates and program sponsors are guided by protocols developed with the advice of Johns Hopkins Geriatrics and Connecticut Community Care, organizations experienced in the evaluation and delivery of services to seniors.
Care Planning and Coordination
The LifeStyle Solutions places a great deal of faith in the effectiveness of its Wellness staff in helping our members maintain their well-being, and in members' use of HomeWorks to enhance independence. But we realize that the day may come when the need for long-term care is unavoidable.
That's why qualified long-term care providers, chosen because of their reputation, expertise and agreement to meet Plan requirements, have been selected to serve you. Just as in the case of your Wellness and HomeWorks Coordinators, your Care Coordinator works with network providers as your personal advocate to help create the most effective plan of care for you.
The Concept of Your Team
Within the retirement services associated with the Plan, your team is made up of your Wellness Coordinator, your HomeWorks Coordinator and, if needed, your Care Coordinator. Each team member works both individually and with other team members to respond to your evolving needs. Trained to respond to the circumstances that develop, they will be there to help you to shape an effective solution to life's challenges.
